Job Description Summary: We are seeking a dedicated Population Health Care Supervisor to support the Director of Care Management in leading our Population Health programs. This role involves planning, coordinating, and overseeing care management activities, collaborating with primary care clinics, and ensuring quality outcomes for patients. Responsibilities: Supervise, train, and evaluate RN/LVN care management staff. Support “Triple Aim” objectives and promote organizational core values. Collaborate with primary care clinics to implement Nurse Navigation and Care Coordination. Oversee care transitions, post-acute home health recertifications, and quality measure documentation. Develop operational workflows within EHRs (Epic Healthy Planet/Compass Rose). Coordinate with Home Health Agencies and ensure HIPAA/compliance standards are met. Assist with reporting, dashboards, and performance management as needed. Lead and support a team of care managers and nurse navigators Drive care coordination, transitions of care (TOC), and home health initiatives Partner with clinical teams to improve workflows and patient outcomes Support population health strategies across value-based programs (ACOs, CINs) What we’re looking for: RN with 5 years clinical experience and 3 years in Case Management Prior leadership or supervisory experience required Background in health plan/managed care (BCBS, etc.) strongly preferred Experience with TOC, home health, and value-based care models BSN preferred We offer a collaborative environment with opportunities for growth and professional development.
