present a rewarding opportunity for a Travel Care Manager RN in
Longview, Texas. This assignment offers a weekly blended rate of
$2,100, based on three 12?hour night shifts. This role is perfect
for an RN with strong clinical judgement, exceptional coordination
skills, and experience supporting patients through complex

Manager RN Perform comprehensive assessments to determine discharge
needs and barriers Develop patient?centered care plans tailored to
clinical, psychosocial, and post?acute needs Coordinate with
providers, therapy, pharmacy, and community resources Facilitate
transitions to SNF, rehab, home health, DME, or outpatient services
Conduct utilization reviews and work with insurance payors Provide
education to patients and families regarding follow?up care
Maintain clear, timely, and compliant documentation Advocate for
patient safety, quality, and continuity of care Requirements of the
Care Manager RN Active Texas RN license or compact eligibility
Current BLS certification Minimum 1–2 years’ Care Management or
Case Management experience Strong interpersonal communication and
