service area of a quarter-million people. Our UT Health Athens
hospital is seeking an experienced RN Case Manager. We are looking
for your experience, knowledge and patient care skills in our Case
Management Department where you will be responsible for: What You
Will Be Doing:
- Responsible for coordinating the care and service of patients
across the continuum promoting quality care, effective utilization
of resources and assuming a leadership role with the
interdisciplinary team to achieve desired clinical and resource
outcomes.
- This is accomplished by assessing, planning, implementing, and
evaluating the delivery of services and working collaboratively
with all members of the health care team.
- Plans and coordinates a comprehensive plan of care using a
concurrent integrated review, standard operating procedures, case
management tools, and collaborative efforts with all members of the
patient's family and/or significant other to enhance quality care
and reduce costs.
- Coordinates patient care to ensure appropriate and effective
clinical outcomes.
- Communicates with third party payers, peer review entities and
physicians' offices to gather and provide clinical and fiscal
information for financial optimization.
- Contributes to a positive work environment and performs other
duties as assigned or directed in order to enhance the overall team
efforts of the department/organization.
Responsibilities
Education:
- Associates or Bachelors' Active Registered Nurse License in
state of TexasExperience:
- Three (3) years of hospital experience required
- Previous case management experience preferred
- Basic personal computer skills and comfort with Microsoft
Windows operating system; database and word processing skills
required
