east Texas. With nearly 7,500 employees and over 1,000 licensed
in-patient beds, UT Health East Texas provides healthcare services
to thousands of patients annually through our hospitals, clinics,
Level 1 trauma facility, air emergency fleet and EMS service in a
metropolitan service area of a quarter-million people. Our Athens
hospital is seeking an experienced RN Case Manager. We are looking
for your experience, knowledge and patient care skills in our Case
Management Department where you will be responsible for: What You
Will Be Doing: Responsible for coordinating the care and service of
patients across the continuum promoting quality care, effective
utilization of resources and assuming a leadership role with the
interdisciplinary team to achieve desired clinical and resource
outcomes. This is accomplished by assessing, planning,
implementing, and evaluating the delivery of services and working
collaboratively with all members of the health care team. Plans and
coordinates a comprehensive plan of care using a concurrent
integrated review, standard operating procedures, case management
tools, and collaborative efforts with all members of the
patient---s family and/or significant other to enhance quality care
and reduce costs. Coordinates patient care to ensure appropriate
and effective clinical outcomes. Communicates with third party
payors, peer review entities and physicians--- offices to gather
and provide clinical and fiscal information for financial
optimization. Contributes to a positive work environment and
performs other duties as assigned or directed in order to enhance
the overall team efforts of the department/organization.
Supervision: The RN Case Manager reports to the Director of Case
Management. Responsibilities Education: Associates or Bachelors---
Active Registered Nurse License in state of Texas Experience: Three
(3) years of hospital experience required Previous case management
experience preferred Basic personal computer skills and comfort
with Microsoft Windows operating system; database and word
processing skills required Strong verbal and written communication
skills Active listening skills Good problem solving skills
Organizational skills. Leadership skills. Ability to manage stress
appropriately and to make decisions under pressure. Patient record
documentation skills Strong time management skills Ability to think
strategically and act tactically Ability to handle multiple tasks
at the same time Ability to embrace the client relationship to
guide decisions and actions Demonstrates good team work
Certification and Licensure: Active RN Licence within the state of
Texas Compliance Requirements Comply with regulatory requirements,
